The Evidence Is Clear

Fitness is vital

Low cardiorespiratory fitness carries a long-term mortality risk comparable to — or greater than — coronary disease, smoking, or diabetes.

Mandsager et al., JAMA Network Open, 2018

12–14 years

Adults who sustained five healthy lifestyle factors at age 50 lived 12–14 years longer — more of them free of major chronic disease.

Li et al., Circulation, 2018

16% higher risk

Each 5 kg decline in grip strength is associated with a 16% higher risk of death from any cause across populations.

Leong et al., The Lancet, 2015

27 hours a day

How long a physician would need to deliver all recommended care for a typical panel. The healthspan layer needs time standard primary care doesn't have.

A different kind of training.
Practice, not theory.

Dr. Corey Sadler is a board-certified family physician who has practiced inside the U.S. Air Force, a federal medical group, and the Veterans Health Administration — managing primary-care panels of more than 1,000 patients and deploying to East Africa as deputy surgeon supporting special operations forces.

He saw the same systemic problem everywhere: medical systems react to illness rather than protect health. Pillar Healthspan is his answer — care that is unhurried, measured, and built around the years of healthy life ahead of every patient.

"I don't ask my patients to do anything I don't do myself."

The Four Pillars

The foundations of lasting health

The greatest drivers of long-term health are often the most foundational. Your care plan is built across all four — measured, tracked, and woven into your personal plan.

Exercise

Cardiopulmonary and strength training as part of primary care — because fitness is one of the strongest predictors of how long and how well you live.

"Fitness is a vital sign."

Steven Blair, Cooper Center Researcher

Nutrition

Daily, weekly, monthly, and yearly nutritional guidance throughout your care plan — food as a genuine part of medicine, not an afterthought.

"Let food be thy medicine."

— Hippocrates

Sleep & Recovery

Longitudinal sleep tracking blended into your visits — so recovery is measured and improved with the same rigor as any lab value.

"Sleep is the golden chain that ties health and our bodies together."

— Thomas Dekker

Healthy Relationships

Longevity's biggest missed factor. People who foster close, meaningful relationships live better, longer — and addressing mental health early protects that capacity.

"Loneliness is a public health concern."

— Vivek Murthy

The Healthspan Reserve Index, tracked over time

One clear number for your physiologic reserve — measured, benchmarked, improved.

A plan across the Four Pillars

Movement, sleep, nutrition, and relationships — with your wearable data reviewed and integrated.

Evidence-based — no fads, no fine print

Covered medical visits and labs still go to your insurance, the way they should.

Real time

A deliberately small panel means longer visits and a physician who actually knows you.

The Healthspan Reserve Index

One clear number for where you stand

The HRI scores your physiologic reserve across six evidence-based domains — from cardiorespiratory fitness and strength to metabolic and cardiovascular risk — and translates them into one number you can track over time.

VO₂max Testing

Gold-standard measurement of cardiorespiratory fitness — the single strongest predictor of long-term mortality.

Body Composition

Precise scan of lean mass, fat mass, and bone density — the tissue-level picture of how you're aging.

Functional Strength Evaluation

Grip strength and functional capacity — validated markers that independently predict survival.

Targeted Lab Panel

The metabolic and cardiovascular markers that genuinely predict how well you'll age — interpreted by your physician.

Member Experiences

Care that members actually feel

A deliberately small panel means every member gets real-time, real attention, and a physician who knows their story.
